Nolia Health, an interdisciplinary healthcare organization specializing in dementia care, caregiver support, and care navigation for older adults living with chronic conditions, and NewDays, a virtual cognitive rehabilitation provider for people living with mild cognitive impairment and dementia, today announced a partnership designed to connect two important parts of dementia care: treatment for the person experiencing cognitive changes and ongoing support for the family caring for them.

Most of what families face after a diagnosis of cognitive impairment happens in between appointments. This partnership is built for that space: NewDays provides a structured cognitive rehabilitation program that turns a diagnosis into an active course of treatment, giving the person experiencing cognitive changes something clinically validated they can begin right away. Nolia provides interdisciplinary clinical support, care navigation and coordination, and caregiver support to help families understand the diagnosis, develop personalized care plans, and navigate the emotional and practical complexities of caregiving.

Together, the organizations are creating a more connected path for families—one that recognizes that treating cognitive changes and supporting the people living alongside them are both essential parts of care.

Treatment and support, together

Cognitive decline affects more than memory. It can change relationships, daily routines, independence, and the roles that families have relied on for years. 

NewDays focuses on the person experiencing cognitive changes, providing clinician-guided cognitive rehabilitation that can be delivered from home. Its program combines regular telehealth visits with licensed cognitive clinicians and daily AI-guided cognitive exercises personalized to the individual. What its clinically validated cognitive rehabilitation offers is time - more independence, more of the experiences and relationships that make someone who they are.

Nolia focuses on the broader care experience: helping older adults and their families understand their needs, coordinate care, develop personalized plans, address behavioral and emotional challenges, and connect with available resources. For eligible families, Nolia also provides support through the CMS GUIDE Model, including care navigation and caregiver support.

The partnership ensures all aspects of a person's care and a family's needs are covered.

What the partnership includes

As part of the collaboration, Nolia and NewDays will:

  • Connect patients and care partners to complementary services at newdays.noliahealth.com, where families can be directed to treatment, family support, or both, based on where they are in the journey.

  • Create a coordinated referral pathway so families can access NewDays cognitive treatment and Nolia's clinical and caregiver support based on their individual needs.

  • Coordinate communication between teams, with appropriate patient authorization, so that treatment and family support can reinforce one another. 

  • Create shared educational and research opportunities: shared data and insights to better understand how cognitive treatment and whole-family support can work together.

  • Improve access to support earlier in the cognitive-care journey: reaching families at the first sign of cognitive changes rather than at the point of crisis.

Why it matters

For families, the experience of cognitive decline rarely fits neatly into one healthcare service. 

The person living with cognitive changes may need treatment and strategies to maintain function. Their care partner may need education, emotional support, help navigating the healthcare system, and practical strategies for managing what is happening at home.
